MyChart is the patient portal that enables you to access your health records, view test results, request prescription refills, and communicate with your healthcare providers. We encourage patients to download the MyChart app on their phone or tablet.

In many cases, CCH medical providers can now access records from other hospitals, clinics, and specialists who utilize Epic.
